The immigration physical (Form I-693) at Instant Urgent Care is a flat $299. This includes the full physical exam, the required blood tests, the TB test, and a review of your vaccination records.

The $299 covers the exam, required lab tests, and review of your records. Any vaccines you still need are charged separately, since the doses required vary from person to person. Bring your vaccination records so we only give what's missing.

No. USCIS immigration physicals are self-pay only and not billed to medical insurance. Payment is collected at the time of your visit.

We accept cash and all major credit and debit cards at the time of service.

No surprise fees. The $299 is the flat exam fee. The only possible add-on is the cost of any vaccines you still need, which we tell you about before administering.

The fee covers the medical examination and completed Form I-693, which is a service performed regardless of your application outcome. USCIS decisions are separate from the medical exam, so the exam fee is not tied to your case result.

All three Instant Urgent Care clinics offer them: San Jose (5138 Monterey Hwy), Santa Clara (3466 El Camino Real), and Sunnyvale (970 W El Camino Real).

Yes — our Sunnyvale and Santa Clara clinics are open Saturday and Sunday, 9 AM to 5 PM. Our San Jose clinic is Monday to Friday, 9 AM to 6 PM.

Most patients are seen within the same week, and often within a day or two. If you have a filing deadline, let us know when you call and we'll do our best to fit you in.

Yes. We can schedule multiple family members together. Let us know how many people need exams when you book so we can allow enough time.

Yes. Patients travel to us from across the Bay Area. Because the exam is completed in a single visit, you only need to make the trip once.

A complete physical exam (eyes, ears, nose, throat, heart, lungs, skin, abdomen, extremities), a mental health screening, required blood tests, a TB test, and a review and documentation of your vaccination records — all in one appointment.

Plan for about 1.5 to 2 hours. If you need vaccinations, allow a little extra time.

USCIS currently requires blood testing for syphilis and gonorrhea for applicants in the required age range. Your civil surgeon will order the tests that apply to you.

Yes, tuberculosis screening is part of the exam. Depending on your history, this is done with a blood test (IGRA / QuantiFERON) or, in some cases, a chest X-ray if further evaluation is needed.

No. It's a standard physical exam plus a blood draw and TB screening. There is no genital or invasive examination involved in a routine immigration physical.

You may be asked to remove outer layers so the provider can listen to your heart and lungs, but it is a standard, respectful clinical exam. Gowns are provided when needed.

Yes. Fasting is not required for the immigration physical. Eat normally and stay hydrated, which also makes the blood draw easier.

The civil surgeon documents findings on Form I-693 as required. Most conditions do not affect immigration eligibility, but certain ones may need follow-up. Your provider will explain any next steps.

USCIS requires proof of immunity to a set of vaccine-preventable diseases, which can include MMR, Tdap/Td, varicella, polio, hepatitis B, influenza (in season), and others depending on your age. Your civil surgeon reviews your records against the current USCIS list.

The COVID-19 vaccine was removed from the USCIS immigration vaccination requirements, so it is no longer required for Form I-693.

Bring whatever records you have. If you're missing required vaccines and can't prove prior immunity, we can administer the needed doses during your visit, or order blood titers to check existing immunity.

Yes. If you're missing any required vaccines, we can administer them at your appointment so your Form I-693 can be completed.

Titers are blood tests that check whether you're already immune to a disease. If you don't have records, titers can prove immunity and may save you from repeating vaccines you've already had.

Vaccine costs are separate from the $299 exam fee and depend on which doses you need. We'll tell you the cost before giving any vaccine.

Yes. Required vaccines depend on age, so children often have a different schedule than adults. The civil surgeon reviews each person's records individually.

Form I-693 is the Report of Immigration Medical Examination and Vaccination Record. It's the medical form USCIS requires for most green card (adjustment of status) applications, and it can only be completed by a USCIS-designated civil surgeon.

Yes. Our exams are performed by a USCIS-designated civil surgeon who is authorized to complete and sign Form I-693. Only a designated civil surgeon can legally complete this form.

Yes. The completed Form I-693 is placed in a sealed envelope. Do not open it — USCIS will reject the form if the envelope has been opened. You submit it sealed.

You submit it. We give you the sealed Form I-693, and you file it with USCIS as part of your application packet (or bring it to your interview, depending on your case).

The civil surgeon must perform the examination to certify the form, so the exam and the form go together. If you had an incomplete or expired exam elsewhere, you'll typically need a new exam with us.

Bring the RFE notice when you call us. In many cases we can address what USCIS is asking for. Let our staff know it's RFE-related so we can prioritize your appointment.

Form I-693 is used for adjustment of status (green card) applicants inside the U.S. Applicants going through a U.S. consulate abroad use a panel physician instead, not a civil surgeon.

The civil surgeon retains required records as mandated. You receive the official sealed copy for USCIS. We recommend you also photograph or copy the form before it's sealed if your provider allows, for your own reference.

Bring a government photo ID or passport, your vaccination/immunization records, any prior I-693 or medical records, a list of current medications, and information about any past or current medical conditions.

Bring a valid government-issued photo ID. A passport works well. The civil surgeon must verify your identity for the form.

It's helpful to bring your USCIS notice or any RFE, but it's not strictly required for the exam. The most important items are your ID and vaccination records.

Just rest, eat normally, and gather your ID and vaccination records. No fasting or special preparation is needed.

Continue taking your medications as prescribed and bring a list of them. Let the provider know during the exam.

Yes. Let the provider know you're pregnant, as some tests (such as certain X-rays or live vaccines) may be adjusted or deferred. The exam can still be completed safely.

For forms signed by the civil surgeon on or after November 1, 2023, USCIS generally treats a properly completed I-693 as valid indefinitely, as long as it was completed correctly. Always confirm current USCIS guidance for your filing.

Many applicants complete it close to when they file their green card application or before their interview. Because validity rules changed in late 2023, timing is more flexible than it used to be — but check current USCIS guidance for your situation.

If no additional testing is needed, the civil surgeon can typically complete and seal your Form I-693 the same day or shortly after, once any required lab results are back.

If a result needs further evaluation (for example, a positive TB screen), the civil surgeon will explain the next step, which may include a chest X-ray or additional documentation before the form is finalized.

With the post-2023 rules, a properly completed I-693 generally does not expire the way it used to. Still, confirm the latest USCIS policy, and avoid long delays between the exam and filing when possible.

Contact us as soon as possible. Because the form must be submitted sealed and unopened, a lost or opened envelope usually means the civil surgeon needs to issue a replacement, which may require another visit.

The civil surgeon documents medical findings objectively on the form. They can explain what was found, but immigration eligibility decisions are made solely by USCIS, not by the medical office.

You receive the sealed Form I-693 for USCIS. For your own records, ask your provider at the visit about getting a copy of your lab results before the envelope is sealed.
