DOT medical exam — questions we hear most

Are your DOT examiners FMCSA certified?

Yes — all our examiners are listed on the FMCSA National Registry of Certified Medical Examiners (NRCME). You can verify our listing at nationalregistry.fmcsa.dot.gov.

Do I need an appointment for a DOT physical?

No appointment required — walk in to any of our 3 Silicon Valley locations. You can also book online above to reserve your time slot and reduce your wait. We serve San Jose, Santa Clara, and Sunnyvale.

How long does a DOT physical take?

Typically 30–45 minutes. To speed up your visit, fill out the first 2 pages of the FMCSA Medical Examination Report (Form MCSA-5875) before arriving. Bring your license, glasses or contacts if needed, and a complete list of your medications.

How long is a DOT Medical Card valid?

A DOT Medical Card is valid for up to 24 months. Drivers with certain medical conditions (e.g., high blood pressure, diabetes) may receive shorter validity periods — 12 months, 6 months, or 3 months — and must return for re-examination sooner.

What do I need to bring to my DOT physical?+

Bring your valid driver’s license, glasses or contacts if you use them, hearing aids if applicable, and a complete list of all current medications. To speed things up, complete pages 1–2 of Form MCSA-5875 before your visit.

What if I fail the DOT physical?

Our providers will discuss your options, which may include treatment for underlying conditions, medical exemptions, or referral to a specialist. Many conditions are manageable — we will give you the correct guidance to get you back on the road.

Does insurance cover DOT physicals?

DOT physicals are often self-pay or employer-paid. Some insurance plans may cover part of the cost. Many employers set up direct billing accounts with us. Call (408) 687-4806 to discuss pricing and billing options
