Is this exam required for you?
All CDL holders must have a valid and active DOT Medical Card per Department of Transportation and FMCSA regulations. You need a DOT physical if you are:
- A commercial truck or bus driver requiring a CDL (Commercial Driver's License)
- A driver whose DOT Medical Card is expiring and needs renewal
- A new CDL applicant who needs initial medical certification
- A driver returning to work after a medical leave or disqualifying condition
- A driver whose certificate validity was shortened (6 or 12 months) and needs re-examination

To reduce your wait time and make your exam as fast as possible, complete the following before arriving at our clinic.
- Fill out the first 2 pages of the
- Medical Examination Report (MCSA-5875)
- prior to your visit
- Have your health history ready — past surgeries, conditions, medications
- Valid driver's license or state ID
- Glasses or contacts (if you use them)
- Hearing aids (if applicable)
- Complete list of all current medications
- Any relevant medical documentation for existing conditions

Typically 30–45 minutes. To speed up your visit, fill out the first 2 pages of the FMCSA Medical Examination Report (Form MCSA-5875) before arriving. Bring your license, glasses or contacts if needed, and a complete list of your medications.
A DOT Medical Card is valid for up to 24 months. Drivers with certain medical conditions (e.g., high blood pressure, diabetes) may receive shorter validity periods — 12 months, 6 months, or 3 months — and must return for re-examination sooner.
Bring your valid driver’s license, glasses or contacts if you use them, hearing aids if applicable, and a complete list of all current medications. To speed things up, complete pages 1–2 of Form MCSA-5875 before your visit.
