The exam includes: complete physical exam (eyes, ears, nose, throat, heart, lungs, skin, extremities, musculoskeletal), mental health screening, blood tests for syphilis and gonorrhea, TB skin test or QuantiFERON blood test, vaccination record review, and required immunizations if missing. Everything is done in one visit.

Form I-693 is currently valid for 1 year from the civil surgeon’s signature date. You must submit it to USCIS while still valid. Do not open the sealed envelope — an opened envelope is considered void. We recommend confirming current validity requirements with your immigration attorney.
