CDL DOT Medical Exams

All Commercial Drivers License holders are required to have a valid and active DOT Medical Card per the Department of Transportation (DOT) and Federal Motor Carrier Safety Administration (FMCSA) regulations.

This examination must be conducted by a medical examiner listed on Federal Motor Carrier Safety Administration (FMCSA) National Registry.  At Instant Urgent Care, our physicals are conducted efficiently and our physicians will give you the correct treatment (if needed) to get you driving on the road.

Getting your DOT Physical has never been easier

Step 1 – Schedule an appointment at our San Jose, Santa Clara, Sunnyvale, Palo Alto, Berkeley, San Ramon or Dublin locations.
Step 2 – Your Medical Exam will be performed by a NCRME certified provider.
Step 3 – We will provide you with your complete DOT Medical Examination Report.

To speed up the process of your visit:
1 – Please fill out the first 2 pages of the Medical Examination Report prior to your visit.
2 – Bring a complete list of all of the following that is applicable to you:
– List of your medications
– Glasses, contacts and any hearing aid
