What is an immigration medical exam?
The immigration medical examination is the health screening USCIS requires from most people applying for a green card or adjustment of status. Its purpose is simple: to confirm an applicant is not inadmissible on health-related grounds. Results are recorded on Form I-693, which only a USCIS-designated physician — a civil surgeon — can complete and sign.
A regular doctor's visit does not satisfy this requirement, even if the same tests are done. The physicians at Instant Urgent Care are USCIS-designated civil surgeons, so your I-693 is filing-ready. It's not a pass-or-fail test of your overall health — it screens a defined, limited set of items: communicable diseases of public-health significance, required vaccinations, and certain mental-health or substance-use conditions. Who needs a Form I-693?
If you're adjusting status or applying for a green card from inside the United States, you almost certainly need an immigration medical exam from a civil surgeon. Here are the most common situations.

- Bring your vaccine records. The single biggest variable is how many vaccines you still need. Proof of prior immunizations, or titer blood tests confirming immunity, may spare you shots you do not require.
- Consider the lab package. Immigration-related laboratory testing may be excluded or denied by your plan, or applied straight to your deductible, potentially leaving you with unexpected bills exceeding $1,000.
- Ask about vaccines and insurance. The exam fee itself is not covered, but many plans do cover some vaccines. We will give you a clear self-pay quote either way.

Please note: the $275 Immigration Lab Package is an optional self-pay service and is not billed to insurance. Lab requirements are subject to applicable USCIS requirements and provider evaluation. The exact tests are determined by the civil surgeon based on your age, medical history, social history and the USCIS and CDC requirements in force at the time of your exam. Not every applicant needs every test — applicants under 15 are generally exempt from syphilis, HIV and gonorrhea blood testing but still complete the physical examination and age-appropriate vaccines.

When to get your exam — and how long it lasts
USCIS rules around Form I-693 changed recently. Getting the timing right helps you avoid paying for a second exam later.
Under current USCIS policy, a Form I-693 signed by a civil surgeon on or after November 1, 2023 is valid only while the application it was filed with (usually your Form I-485) remains pending. If that underlying application is later withdrawn or denied, the I-693 is no longer valid, and you would need a new exam for any future filing. Exams signed before November 1, 2023 follow the older rule and remain valid for two years from the civil surgeon's signature date. Because these rules can change, always confirm the latest requirements with USCIS or your immigration attorney.
USCIS only accepts the current 01/20/2025 edition of Form I-693. Using an outdated edition is one of the most common reasons a medical exam gets rejected. Our civil surgeons always complete the current accepted edition for you. . Since December 2024, USCIS has generally required applicants to submit Form I-693 together with Form I-485 rather than sending it in separately later. Your attorney can confirm exactly how your packet should be assembled.
Because validity is now tied to your pending application, it usually makes sense to schedule your exam close to when you plan to file, not months in advance. Aim to leave enough time for the TB skin-test follow-up read (48–72 hours) and any vaccines that need a series. . Your completed I-693 is placed in a sealed envelope. USCIS will reject a medical exam if the envelope has been opened or tampered with, so leave it sealed and submit it exactly as you received it. . We'll also give you copies of the relevant pages for your own records, so you have your vaccination history without breaking the seal on the official form.

Form I-693 (Report of Medical Examination and Vaccination Record) is the USCIS-required medical form for green card applicants and others adjusting immigration status in the United States. It must be completed by a USCIS-designated civil surgeon — which our provider is. You submit the sealed form directly to USCIS; do not open the envelope.
The exam includes: full physical examination (eyes, ears, nose, throat, heart, lungs, skin, extremities), mental health screening, blood tests for syphilis and gonorrhea, TB skin test or QuantiFERON blood test, review of vaccination records, and required immunizations if any are missing. All completed in one visit.
Plan for approximately 1.5 to 2 hours at your appointment. If you need missing vaccinations or additional blood work, allow extra time. Bring your vaccination records to avoid unnecessary repeat testing.

Form I-693 is currently valid for 1 year from the date of the civil surgeon’s signature. USCIS must receive the sealed envelope before the form expires. Do not open the sealed envelope — it will be voided if unsealed. We recommend confirming current validity requirements with your immigration attorney
Bring whatever records you have. If you are missing vaccinations that USCIS requires, we can administer them on site during your visit. Your civil surgeon will document everything on the I-693 form.
